Social Security: Filing for disability just got easier
Social Security has begun sending a starter kit which includes a fact sheet and medical worksheet for your convenience when you request an appointment to file for disability benefits. It’s another step Social Security is taking to ensure satisfaction when it comes time to filing for benefits.
The fact sheet explains how Social Security decides if you are disabled, how long it takes to get a decision and how we make the decision. The fact sheet also gives you a lot of other useful information such as what type benefits you can receive, is your personal information safe and the available Web site for questions.
The medical worksheet helps you prepare for your disability interview or to complete the Disability Report on the Internet. The information we will need include the name, address, phone and patient ID number for any doctor, hospital or other facility in which you may have received treatment.
Information about your work history for the last 15 years is also needed for our decision.
This package reminds you what additional documents you will need for your disability interview. Needed are your birth certificate, most recent W-2 form, Social Security numbers for all persons involved, military discharge papers and proof of your income.
